Human Growth Hormone Testing Methods

Previously, human growth hormone was tested using a blood test, which can detect the presence of the synthetic hormone, but there is however, only a 48 hour window from the time of injection.

A new testing method is much better, because it is able to detect human growth hormone use up to 14 days after it has been injected. This new technique is the same one used to detect breast and bone cancer. More about that later.

Up until the most recent testing method the following HGH tests were used:

  • Somatotropin hormone test
  • Somatomedin C or IGF-1 test
  • GH stimulation test, insulin tolerance test or Arginine test
  • GH suppression test or glucose loading test

Somatotropin Hormone Test

The somatotropin hormone test involves taking a blood sample 1-2 hours after sleep. It will determine whether there is a deficiency or an excess level of HGH in the body. If human growth hormone levels are determined to be higher than what is considered normal levels, which are different in men, women and children, it indicates that further tests must be made to determine the causes behind this. If the levels are lower than normal, then an HGH deficiency exists.

Somatomedin C Test

Somatomedin C or insulin-like growth factor test (IGF-1 test) is also a blood test and workouts, food intake and time do not affect it. In adults, the normal level of IGF-1 is between 42 to 110 ng/ml. In children under the age of 8, the level is between 7 to 110 ng/ml in girls and 4 to 87 ng/ml in boys. In children that are 9-10 years of age the level is between 39 to 186 ng/ml in girls and 26 to 98 ng/ml in boys. In children of that are 11-13 years, the level is between 66 to 215 ng/ml in girls and 44 to 207 ng/ml in boys.

GH Stimulation Test

The GH stimulation test is also called the Arginine test. In the GH stimulation test, arginine is injected into the blood in order to stimulate the production of growth hormone by the pituitary gland. After the injection the release of the GH is measured. A value that is less than 10 ng/ml, means there is a deficiency in the secretion.

GH Suppression Test

The GH suppression test, also known as the glucose loading test first measures the baseline level of the hormone. Then 2 blood samples are taken. The first is taken before a glucose ingestion and the second is taken after a glucose ingestion. The normal result of the test is 3 ng/ml. If there is a deviation from what is considered the normal value an abnormal condition is indicated.

Testing Limitations

Synthetic growth hormone injections are difficult to identify. The current testing methods are considered state-of-the-art and they still are unable to detect HGH more than 48 hours after the injection. In addition, these testing methods cannot detect pharmaceutical HGH releasers.

Peter Sonksen, a professor of endocrinology and an HGH expert, who is at St Thomas' Hospital and King's College in London, says he received a grant from the IOC in 1999 to develop a synthetic HGH test, He says he has formulated that test but the WADA and IOC are not using it.

HGH-releasing drugs don't contain any human growth hormone. They work by helping the body to increase its natural production of HGH. As a result, HGH releases cannot be identified with any of the current testing method being used.

These current HGH testing limitations mean that even though pharmaceutical HGH releasers are on the list of banned substances by WADA, it means very little. Gary Wadler, who is the chairman of WADA's Prohibited List and Methods Subcommittee, an associate professor at New York University medical school and runs a private practice in internal medicine and sports, says athletes likely know this.

"We know that there are athletes out there that follow these things closely. The first question they'll ask is if there's a test for it," said Wadler.

The HGH test that is currently used was introduced prior to the 2004 Summer Games. To date it has not turned up a positive result for the substance in any athlete. None of the U.S. pro sports leagues utilize HGH blood testing or any other tests for HGH. Sonksen says his HGH test would be far more effective against HGH releasers and synthetic HGH.

Sonksen says, "Only the world's dumbest athlete would be caught by the current test because of its known time limitations.”

The Most Recent HGH Testing

This test is similar to a test that is used in cancer treatments. Antidoping officials are excited that a new method of catching athletes using HGH is going to be available and that this method is highly effective.

This is the same test that is used to detect breast and bone cancer. The laboratory blood sample is taken and centrifuged. Then certain chemicals are added and an instrument determines the intensity of luminosity in the blood sample. The measure of light intensity is able to determine when the human growth hormone was administered.

Antidoping experts say, The light’s intensity, signals whether the person has used human growth hormone in the past 10-14 days. The name of this technique is the Biomarker’s test, which has the ability to identify chemicals in the body that generally increase when synthetic HGH levels go up.

It’s not secret - antidoping officials are always anxious to share any new testing methods, while skeptics often accuse them of exaggerating the scientific developments. However, this time may be quite different. The officials stand by the biomarkers test and say that this test is a significant advancement over the current HGH testing methods, because this test is able to detect whether HGH has been used in the previous 24-48 hours.

World Anti-Doping Agency officials oversee the testing protocols of Olympic athletes along with numerous professional athletes outside the USA. Their response to this new test is that after over a decade of research, the biomarkers test is just months from being utilized on athletes.

When WADA introduces the test it will likely strengthen the debate over the use of HGH blood tests that is currently happening in the NFL and Major League Baseball. In fact, just recently the debate really intensified, after a professional rugby player in England was suspended after he tested positive for HGH. This was the first time any athlete had been suspended for an HGH positive reading in the past 6-years of testing and officials in the NFL and baseball league where clearly affected.

As a result union officials from the NFL and Major League Baseball unions have expressed their concerns about the current test. They are quick to point out this test has only caught one athlete. In addition, they caution there have never been blood tests for performance-enhancing drugs in either of these leagues.

Even as the great HGH debate carries on, it is clear that the science behind HGH testing has progressed.

The WADA general director, David Howman, says he believes the biomarkers test would make it more difficult for anyone in Major League Baseball or the NFL to argue that no viable tests for HGH existed.

Howman says, “The other test is scientifically reliable, and so is the new one. Anyone who tells you otherwise hasn’t looked at the science.”

According to the test creators they set the threshold for a positive, particularly high, similar to the other levels of tests used by WADA officials.

HGH naturally occurs in the body. It is believed that HGH injections decrease body fat and increase lean muscle growth. There have been numerous instances where HGH has been used by athletes who have suffered injuries and HGH has been used as part of their rehabilitation.

In the USA it is illegal to possess HGH, unless you have a prescription, and it is banned for use as a performance-enhancer in all major sports around the world. Howman says since the 1970s athletes have been using human growth hormone with great impunity. It is readily available to athletes who choose to freely use it and they aren’t likely to continue to use something that does not help.

The difference between the biomarkers test and current tests is that the biomarker test detects chemicals in the body that are raised when HGH levels spike whereas the current tests simply detect the presence of synthetic HGH. Antidoping officials believe in the future, both tests will be used, because they take different measurements.

The chief science officer for the US Anti-Doping Agency, Larry Bowers, says, “The tests are complementary. The biomarkers test can’t detect use in the first 48 hours, so that is why you use both of them. It’s another tool for us to use.”

Usada’s Travis Tygart, says, “It’s obviously very significant and it is why over the past several years, we have invested money in not just the direct test but the new marker test. We knew that the two of them would provide a significant deterrent.”

The top drug testing official in the NFL, Adolpho Birch says, Our advisers keep us informed on the scientific progress and we are in regular contact with Usada and WADA officials,” Birch said. “The Partnership for Clean Competition, a research consortium that we founded along with other partners, recently held a conference at which the state of the research in this area was thoroughly discussed.”

It would appear that we may see some changes to the HGH testing in professional sports sometime soon. How soon remains to be seen.
